Sparta Community Hospital Mobile Health Clinics will be hosting a clinic for sports physicals on May 6 and May 9, 2024. Please note that an appointment must be scheduled prior to those dates by calling 618-443-1DOC (1362). Also, parents must sign and complete the physical form in order for the athlete to be seen by the doctor. This is only for sports physicals. This DOES NOT replace a school physical that is required for upcoming 6th and 9th graders for the 2024-2025 school year. Please see attached flyer for more details.
