The Sparta Mobile Clinic will be offering sports physicals on April 20th and 21st. Appointments are required and can be made by calling 618-443-1DOC (1362). A form needs to be filled out and signed by parents or guardian ahead of time. Please scan the QR code or visit the website to get the form. The cost is $10 and will be donated to Trico School. This does NOT count as a 6th grade or 9th grade regular health physical. Please see flyer for additional information.
